Personal Information of Individual Who Will Be Sending a Card or Gift to a Cancer Fighter

For the safety and privacy of our cancer fighters as well as our individuals who will be sending gifts, we do require that some basic information be submitted before we are able to release the personal information of a cancer fighter. Please be assured that your information will never be shared with any other entity and will be used solely for the purpose of gifting a cancer fighter.
